International Pet Travel Information

If you will be travelling internationally with your pet, you will need to obtain an International Pet Health Certificate, also known as the USDA APHIS Form 7001. This health certificate can only be obtained by having your pet examined by a USDA-accredited veterinarian. All veterinarians at Hyde Park Veterinary Clinic are UDSA-accredited.
